2015 Community Wellness Report
This report is intended to be used by community-based Primary Health Care Provider Teams, Clinic Teams , action teams, working groups, KTHR staff and community leaders to inform and to develop strategies to address unmet needs and emerging issues in the community of Kelvington.
In Primary Health Care we are committed to a health car system that puts patients first by providing Better Health, Better Care, Better Values and Better Teams for our residents.
The Community Wellness Assessment Survey was offered to Kelvington and Area residents both online and in person at East Central Co-op Marketplace in May 2015. 120 surveys were completed.
Next Steps:
The Kelvington Health Action Team will be reviewing the survey results and developing strategies to meet some of the unmet needs. The team will also bring the report forward to community leaders such as Mayors, Town Councils and Interagency Committee.
